Once we get to know you and understand what’s important to you, we’ll provide everything you need to know and more.

Step 2: Review your financial and funding options

We can help to access funding options including the Commonwealth Home Support Program (CHSP) or a Home Care Package (HCP) via myagedcare.gov.au. As this process can take time, please call us on 1300 487 104 if you require services urgently or have had a health incident. We can also help if you can’t or don’t want to access government funding through our self-funded retiree service options.

Design your care plan

Our dedicated care advisors will guide you through it all. Once we have your funding choice decided, our compassionate team will work with you to create a Care Plan that matches your lifestyle, interests and needs.

Step 5: Receive services

Once you are happy with your Care Plan and have signed your Services Agreement, we will book your services with our Catholic Healthcare team and preferred providers.

  • We understand there’s no place like home. That’s why we offer a wide range of in-home aged care services so you or your loved one can remain living independently at home. From making daily tasks easier with transport to the shops, cooking or cleaning to improving health and wellness, clinical services and more, we’re here to help you maintain your lifestyle at home for longer.

    Home Care Services (also commonly referred to as in-home care) can be Government subsidised under either the Commonwealth Home Specific Programme (CHSP) for basic level care, or Home Care Packages (HCP) for multiple or more complex requirements.

    Designed for individuals with basic care needs. 

    Provides a lower level of care and support, including assistance with basic activities of daily living. 

    Level 2: 

    Suitable for individuals with low-level care needs. 

    Offers a bit more comprehensive support compared to Level 1, including assistance with personal care and some allied health services. 

    Level 3: 

    Designed for individuals with intermediate care needs. 

    Provides a higher level of support, including more complex care services and specialized health services. 

    Level 4: 

    Tailored for individuals with high-level care needs. 

    Offers the highest level of support, including a wide range of services to meet complex care requirements.

  • A Home Care Package provider is an accredited organisation approved by the Australian government to deliver aged care services. They play a vital role in providing personalised care and support to help older Australians live independently at home.

    When your Home Care Package (HCP) is approved, the funds don’t go directly to you. Instead, an approved HCP provider, also known as a ‘Home Care Package fund manager,’ holds the funds and manages payments for services on your behalf. You have the freedom to choose a provider that best suits your needs.

    Their responsibilities include developing a care plan, managing funds, monitoring and reviewing care, and ensuring compliance with aged care regulations.

  • There are many approaches we take to ensure the right Care Worker is matched to you depending on your needs and preferences. We employ Care Workers based on their qualifications, skills, experience and alignment with our company’s mission and values.  We also have case management expertise and ongoing reviews of your care needs by our experienced Care Advisors to ensure you are maintaining your health, independence and lifestyle at home for longer.

  • My Aged Care allocates Packages based on date of approval of HCP and priority level.  

    For people with a medium priority, the expected wait time for an approved Home Care Package (starting from the date on the letter of approval) is: 

    • Level 1: 3 - 6 months 
    • Level 2: 6 - 9 months 
    • Level 3: 9 - 12 months 
    • Level 4: 12 - 15 months
