Home Care Packages (HCP) and steps to Accessing One

With more than 18 years of experience in providing Home Care support, we offer flexible Home Care Packages that take into account your individual lifestyle, cultural background and religious beliefs.

Care in the comfort of your own home

Some things can get a little harder to manage later in life, but with the right services and support, you can remain living independently for longer.

We can help with everything from household chores to more personal care, and assisting with transport needs. Our carers can drive you to visit family and friends and attend appointments and events.

The great thing about in-home support is that you get to stay living in your own home, doing all of the things you want to do.

Home Care Packages at a glance

Government funded packages are available to those assessed as being eligible for the Home Care Packages (HCP) program.

  • The HCP program provides packages to support people with more complex needs living at home
  • HCP includes clinical and nursing services, respite services, social support, help at home, transport options, personal care, meal services and equipment
  • HCP enables you to choose your preferred range of services to meet your individual needs.
  • HCP provides scope for personalised and targeted care services
  • Your local Aged Care Assessment Team (ACAT) member will guide you through the assessment process to determine your  eligibility for a package
  • You can request the ACAT to refer you to Catholic Healthcare’s Community Services

To get started, you need to register with My Aged Care and be assigned a package.

Getting started

We can help you with advice on how to access Government funded services such as a Home Care Package.

Call us and we will help you to navigate through the process, starting with registering with My Aged Care. Have your contact details ready including your address, contact number, Medicare number and pension number.

Assessing your eligibility

A member of your local assessment team will guide you through the process to determine your eligibility for a Home Care Package and what level of Package is best for you. Your eligibility is determined by your current situation and the types of care and services you require.

Building your plan

We want to work with you, to support you and provide the help and advice you need.

Your needs and preferences may change over time. To provide you with flexibility and personally-tailored care, your care plan will be reviewed regularly by your Catholic Healthcare advisor, to ensure you receive the level of support you need to keep you safe, happy and independent.

Choosing your services

Working through your goals, motivations, interests and personal preferences will help you make informed choices about the services and assistance you would like to receive moving forward.

The services you choose will form the basis of your service agreement and care plan.

When you are being assessed for your service options, ensure you ask for Catholic Healthcare. We will assist you to find the service that suits your personal needs.

Finalising your plan

Your personalised budget is a specified amount that the Australian Government provides for each person’s Home Care Package. The Aged Care Assessment determines the level of support you are eligible to receive.
