Once we get to know you and understand what’s important to you, we’ll provide everything you need to know and more.
Step 2: Review your financial and funding options
We can help to access funding options including the Commonwealth Home Support Program (CHSP) or our Support at Home program (formerly Home Care Packages) via myagedcare.gov.au. As this process can take time, please call us on 1300 487 104 if you require services urgently or have had a health incident. We can also help if you can’t or don’t want to access government funding through our Self-funded services option.

Switching is simple — we’ll help you transition smoothly from your current provider to Catholic Healthcare.

Design your care plan
Our dedicated Care Partner teams will guide you through it all. Once we have your funding choice decided, our compassionate team will work with you to create a Care Plan that matches your lifestyle, interests and needs.
Step 5: Receive services
Once you are happy with your Care Plan and have signed your Services Agreement, we will book your services with our Catholic Healthcare team and preferred providers.

What is Home Care?
We understand there’s no place like home. That’s why we offer a wide range of in-home aged care services so you or your loved one can remain living independently at home. Home Care Services (also commonly referred to as in-home care) can be Government subsidised under either the Commonwealth Home Support Program (CHSP) for basic level care, or Support at Home program (formerly Home Care Packages) for multiple or more complex requirements. -
