The Catholic Healthcare Home Care Services App makes managing everyday life easier for clients and their loved ones, putting care and support right at your fingertips. Free and easy to use, it gives you greater control through a simple self‑service experience.
And we have great news! With the latest version of the app, you can now explore Catholic Healthcare’s full range of services and easily request and manage the support that helps you stay independent at home, all in one place!
It is now easier than ever to manage your services
To access your services, or you need to do is go to the ‘Manage Services’ tile. Here you will be able to see a list of popular services, and view, request or change services.
Popular services
- Check your upcoming schedule including dates, times, and which community worker will be supporting you.
- Request new services (for example, transport to a medical appointment) or make changes to your existing ones.
View or request services from the full-service catalogue
- Book allied health services such as physiotherapy, occupational therapy, and nursing support as needed.
- Sign up for Mind+Move activities including exercise, art classes, and social outings, to stay active and connected in your community.
- Securely access important documents, like your care plan, service agreements, and invoices all in one place.
- Stay up to date with the latest news and events happening at Catholic Healthcare.
- If you need to reschedule or cancel, you make the changes in this app. Simply hit the Manage Services icon and select Change or Cancel a Service.
For existing Catholic Healthcare clients, to request a service, all you need to do is tap the Request a Service icon. Then select the category, for example allied health, and then select the specific service, such as physiotherapy.
Once a request has been submitted, one of our friendly Customer Service team members will contact you within 24 hours to discuss your request. It really is that easy. Even if you don’t have funding for a particular service, there’s no need to worry, you can still request it via the app and pay for the service in full.
