At Catholic Healthcare, we believe that staying active and independent is essential for living well. That is why our in-house physiotherapy team is here to support you—either in the comfort of your own home or in retirement living.
What Do We Offer?
Our physiotherapists provide personalised care tailored to your needs. Whether you are recovering from an injury, managing pain, or want to stay mobile and strong, we are here to help.
Our services include:
- In-home physiotherapy for HCP clients across Sydney Metro and the Central Coast
- Clinic-based sessions at Catholic Healthcare's Independent Living Units for CHSP clients
- Functional assessments to evaluate mobility, balance, strength, gait, and fall risk
- Individualised rehabilitation programs and exercise prescriptions
- Pain management strategies
- Mobility aid recommendations
- Falls prevention education.
We also offer services to clients who wish to purchase additional physiotherapy outside of their funded programs.
Who Can Access Our Services?
- CHSP clients living at home or in Independent Living Units (subject to availability).
- HCP clients across the Sydney Metro and the Central Coast.
- Restorative Care Program clients under the new Support at Home model (from 1 November 2025).
Our services will be available to CHSP and Support at Home clients from 1 November 2025, ensuring continuity of care under the new aged care reforms.
How Does It Work?
You can access physiotherapy through your Care Advisor or Care Partner team, who will help coordinate your referral. No GP referral is needed. Once referred, our team will:
- Review your referral and place you on the waiting list
- Schedule your assessment at home or in the clinic
- Bring any necessary equipment for your session
- Provide handouts and guidance for home exercises
- Offer follow-up sessions and reassessments as needed.
We also work closely with other allied health professionals—such as Occupational Therapists, Home Organisers, and Registered Nurses—to ensure your care plan is holistic and current.
A Case Study: Sally’s Journey to a Pain-Free Shoulder
Sally, one of our HCP clients, mentioned to her Care Advisor that she was experiencing shoulder pain. Her Care Advisor suggested a referral to the Catholic Healthcare physiotherapy team. Sally agreed, and the referral was submitted through our system.
Soon after, Sally was contacted by our rostering team and scheduled for an in-home assessment with Hayley, one of our physiotherapists. During the visit, Hayley assessed Sally’s shoulder, explained her condition, and prescribed a tailored exercise program. She also recommended some simple equipment that could be purchased using Sally’s funding.
Hayley visited Sally weekly over the next four weeks to guide her through her exercises. Sally also followed her home program using the handouts provided. By the end of the sessions, Sally reported significant improvement and no longer felt pain. Hayley updated Sally’s Care Advisor and recommended a review in three months to maintain her progress.
Ongoing Wellness Support
Our Allied Health Assistants also deliver in-home health and wellness programs designed by our Lead Physiotherapist. These programs support ongoing rehabilitation and maintenance, helping you stay strong and independent.
