Starting 1 November 2025, this program is part of a major update to aged care in Australia. It’s designed to offer more flexible, personalised care so you or your loved one can stay independent, safe, and comfortable at home.

Is it time to consider Support at Home?
There’s no one right moment, but here are a few clues that it might be time to explore in-home support:
✅ Everyday tasks are getting harder
If cooking, cleaning, showering, or managing medications are becoming tricky, it might be time to get some help.
✅ You’re worried about safety at home
Struggling with stairs? Worried about falls? Feeling safe and confident in your own space is so important — and support services can help.
✅ You want to stay independent, just with a little help
Support at home is all about helping you keep your independence while making life a little easier.
✅ You or your carer are feeling stretched
Caring can be rewarding — and exhausting. If you or your carer are feeling overwhelmed, bringing in extra support can make all the difference.
What’s included in the Support at Home Program?
This new program replaces the old HCP and CHSP systems, combining everything into one easier, more streamlined approach.
Services might include:
✅ Personal care (like help with showering or dressing)
✅ Housekeeping and home help
✅ Meal prep and grocery support
✅ Social connection and companionship
✅ Transport to appointments or outings
✅ Clinical care (nursing, medication support, physio, etc.)

How do I get started?
The process is simpler than you might think:
1. Register with My Aged Care
Give them a call on 1800 200 422 or visit myagedcare.gov.au.
(Tip: As of 9 December 2024, there’s now one easy assessment process called the Single Assessment System — so no more juggling ACAT, RAS and others.)
2. Get assessed at home
A trained assessor will visit you and chat about your needs. This helps determine what level of care you’re eligible for — either at home or in a residential care home.
3. Choose your care provider
Once you’re approved, you can pick a provider — like Catholic Healthcare. We’ll guide you through the rest and help tailor a plan just for you.
4. Start receiving care
You’ll begin receiving services at home — everything from help with meals and showering to social visits and more.
