We can provide support in the home in a number of ways:
Home Care Packages (HCP): Government funded packages are available to those assessed as being eligible for the Home Care Packages (HCP) program. To get started, you need to register with My Aged Care and be assigned a package.
Our team can connect you with My Aged Care and facilitate your registration with My Aged Care.
Fee For Service: If you don’t wish to have an ACAT assessment, we can provide ad hoc or ongoing services at an agreed cost. Please contact our team on 1800 225 474 for further information or details.
Veterans’ Home Care (VHC): support is available to veterans and widows/ers through a referral from the Department of Veterans’ Affairs (DVA). You will need to contact the DVA in the first instance.
Commonwealth Home Support Program(CHSP):
The Commonwealth Home Support Programme (CHSP) is a program introduced by the Australian Government to help the older people live more independent lives and stay living within their communities. The CHSP is available for older Australians who can mostly manage on their own but may, from time-to-time, need short term help or basic assistance with some activities.
It is important to note that a Commonwealth Home Support Programme is different to a Home Care Package in that it will only offer very low-level, basic support. If you require a greater level of support you should consider a Home Care Package.
The Commonwealth Home Support Programmes differ from Home Care Packages in their eligibility criteria. With a Home Care Package there are no age restrictions but with Commonwealth Home Support Programmes you must be:
- 65 years or older (50 years or older and identify as an Aboriginal or Torres Strait Islander person) or
- 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless or at risk of being homeless.
To get started on your home care journey please contact us and we will walk you through the My Aged Care, the single point of access into the aged care support system.
When your assessment is complete Catholic Healthcare will work with you to develop tailored Commonwealth Home Support Programme supports.
