Home Care Packages are subsidised by the Federal Government and to be eligible for a package you must be assessed by an Aged Care Assessment Team (ACAT).
An ACAT assessment involves a visit to your home usually by a nurse, social worker or other health professional that comes and talks to you about how well you’re managing in your daily routine.
This assessment includes identifying your care needs and whether a home care package can support you at home, helping you to live the life you want to live.
